Voluntary Registration

I hereby confirm my participation as a volunteer in the activities of Construyendo Comunidades Integrales A.C. I declare that I am fully aware of the implications of my participation and certify that I am in good health to perform the required activities. In the event of an accident during the volunteer days, I authorize Construyendo Comunidades Integrales A.C., the organizers, and those responsible for the event to provide the necessary first aid and, if required, to transport me to the appropriate medical facility. I agree to comply with the guidelines, policies, procedures, and regulations of Construyendo Comunidades Integrales A.C. to ensure the safety of volunteers, beneficiary families, and any other individuals involved in the activities. Furthermore, I grant the broadest possible legal release to Construyendo Comunidades Integrales A.C., its organizers, and those responsible for the event, expressly waiving any civil, criminal, or other claims in the event of an accident or injury during my participation.

By this document, I, as the parent or legal guardian of the minor(s), authorize their participation as a volunteer in the activities organized by Construyendo Comunidades Integrales A.C., and I declare that I am fully aware of the implications of such participation, certifying that the minor is in optimal health to perform the required activities; Furthermore, in the event of an accident during the volunteer days, I authorize Construyendo Comunidades Integrales A.C., its organizers, officials, and third-party partners to provide the necessary first aid and, if required, to transport the minor to the appropriate medical facility for treatment; I commit to ensuring that the minor complies with the safety guidelines, policies, procedures, and regulations established by the association, and I grant the broadest possible legal release to Construyendo Comunidades Integrales A.C., its organizers, officials, and partners, expressly waiving any civil, criminal, administrative, or other claims arising from accidents or injuries that may occur during the child’s participation, provided that actions are taken in accordance with the established guidelines.

I authorize Construyendo Comunidades Integrales A.C. to use my image in promotional materials, both in print and electronic media, within and outside the country. I declare that this authorization is voluntary and provided free of charge. In accordance with the Federal Copyright Law, I permit the reproduction, transmission, and recording of my image in videos, graphics, texts, and any other promotional material related to volunteer work. I may revoke this authorization at any time by submitting a written request to Construyendo Comunidades Integrales A.C.

As the parent or legal guardian of the minor, I hereby expressly, voluntarily, and without compensation authorize Construyendo Comunidades Integrales A.C. to use the minor’s image in promotional, informational, and institutional materials, both in print and electronic media, within and outside the country; in accordance with the Federal Copyright Law, I permit the reproduction, transmission, and recording of said image in videos, graphics, texts, and any other promotional material related to volunteer activities, and I hereby state that I may revoke this authorization at any time by submitting a written request to Construyendo Comunidades Integrales A.C.

This document provides Medical Expense Insurance with the following coverage:
•⁠ ⁠Accidental Death: up to $500,000.00
•⁠ ⁠Organ Loss: up to $500,000.00
•⁠ ⁠Reimbursement of Major Medical Expenses Due to Accident: up to $200,000.00, with no deductible.
Eligibility and Term Conditions:
•⁠ ⁠Age of acceptance: 6 to 69 years old.

I hereby declare that I am aware that this document provides coverage for the minor under an Accident Medical Expense Insurance policy purchased by Construyendo Comunidades Integrales A.C., with the following coverage: accidental death coverage up to $500,000.00, coverage for loss of limbs up to $500,000.00, and reimbursement of medical expenses due to an accident up to $200,000.00 with no deductible, applicable in accordance with the conditions established by the insurer, with an eligibility age of 6 to 69 years, and coverage valid exclusively during the course of official volunteer activities.
